QUESTION: For waivered services, how do other providers check the client’s MARs? Is there anything in place that allows nurses to complete MAR reviews when the clients are not present in the homes? Can all of the clients' MARs in each home be kept in the same MAR book that remains locked in the medication cabinet? (A copy of the monthly physician orders with all medications listed would still be kept in each individual binder.). They are checked on an ongoing basis by staff in the homes. Also, the Home Managers do spot checks throughout the month and deal with any issues they find. Nurses check them at the end of the month and also occasionally when they are in the home. I don't know of anything that doesn't allow it. However, we would avoid having agency employees in the client's home when they aren't there. 460 IAC 6-17-3 only specifies what records providers have to keep at the site of service delivery. It does not specify how they are kept. However, it would be difficult to maintain confidentiality if Case Managers or guardians want to see information for their client/family member if that information is kept in the same binder as other clients' information..
