QUESTION: Would any provider agencies be willing to share their nursing assessment forms (monthly, quarterly, DISKUS)? Do other group home providers have their nursing personnel do monthly assessments for all consumers being served or just those with high medical needs as identified by the consumer’s team? Our agency’s group home staff complete a physical assessment form monthly that triggers the need for other assessments. We also utilize Care Tracker, which alerts agency personnel daily to many things, such as no bowel movement has occurred that day.
