QUESTION: Do other Waiver providers have a criteria/criterion for when a self-medicating client makes med errors frequently enough to determine they need a staff person to pass their medications? While acknowledging that it depends on the individual and his/her IDT, we would be interested to know if providers move forward with that process once a certain number of med errors occur over a certain period of time. To date this has been handled on a case by case basis by each individual IDT. Could also utilize the Human Rights Committee. No, we don't have written criteria, but generally would look to make a change if more than 3 errors occur in a year. We also add staff peer review of client self admin.
