Urgent Reminder for Aged and Disabled Waiver & Traumatic Brain Injury Recipients and Their Families 


This is an important and time-sensitive reminder for Aged and Disabled and Traumatic Brain Injury waiver members and their families. If a legally responsible individual is providing attendant care to the waiver member, the deadline for submitting a transition plan with your care manager is June 15, 2024. 

FSSA announced in January that waiver members who receive attendant care provided by a legally responsible individual would need to transition to an alternative provider or to Structured Family Caregiving by July 1.  

In order to have new service plans approved by July 1, those plans must be developed with your care manager, who will then submit it to the state, by June 15.

In some instances, families have not indicated whether they will continue to receive attendant care provided by an individual other than an LRI or transition to structured family caregiving. It is essential you communicate your choice to your care manager as soon as possible if a plan is not underway. LRIs will not be permitted to deliver attendant care beginning July 1. 

If you have not heard from your care manager, or are unable to reach your care manager, please let us know so that we can help. Please send an email to medicaid@fssa.in.gov and include relevant details and your phone number so that we can reach you.  

If you have missed an outreach from your care manager, please follow-up with them as soon as possible. Care managers have been equipped to provide you with information to inform your decision-making and to update your service plan for a July 1 effective date. 

As previously announced, a structured family caregiving tier for individuals under 18 will be determined based on approved attendant care hours during this transition period only. FSSA has completed entering information in the care management system for each waiver recipient under 18 with an LRI providing care, so your care manager can add SFC to your service plan at this time if this is the transition you choose. After July 1, any family who selects structured family caregiving will have their tier determined using the SFC assessment tool. As noted earlier, more detail on the transition choices and process is available in the May announcement, and additional resources are available on the Medicaid Strategies page.
